SOCIAL SECURITY SYSTEM REQUIREMENTS

Employees who should apply are stated above as provided by the Social Security System, Philippines.

a. A private employee, whether permanent, temporary or provisional, who is not over 60 years old.

b. A household-helper earning at least P1,000 a month is compulsory covered starting Sept. 1, 1993.A household-helper is any person who renders domestic services exclusively to a household such as driver, gardener, cook, governess, and other similar occupations.

c. A Filipino seafarer upon the signing of the standard contract of employment between the seafarer and the manning agency which, together with the foreign ship owner, act as employers.

d. An employee of a foreign government, international organization or their wholly-owned instrumentality based in the Philippines, which entered into an administrative agreement with the SSS for the coverage of its Filipino workers.

e. The parent, spouse or child below 21 years old of the owner of a single proprietorship business.

Documents Required

1. Duly accomplished Application for Driver's License.
2. Proof of identity - Original Birth Certificate with photocopy
- NSO authenticated Birth Certificate with Official Receipt and photocopy
- Original valid Passport with photocopy
- Any legal or government issued document to prove his age and identity (SSS ID, PhilHealth ID, GSIS ID, TIN ID, Pag-ibig ID, Office ID)

Additional Requirements for Minor Applicants (16-18 yrs. old)

1. Letter of Parental/Guardian Consent duly notarized


Additional Requirements for Foreigners

1. Original Alien Certificate of Registration (ACR) with photocopy

2. Original valid Visa/Passport with photocopy and any of the following:
- Permit to stay in the country for at least five (5) months and has stayed in the country for at least one (1) month

- Contract from Philippine employer that certifies stay of at least five (5) months

Additional Requirements for Minor Foreigners (16-18 yrs. old)

CONDUCTOR'S LICENSE: Qualifications and Requirements

5. Must be clean, neat and presentable.

Requirements :

2. Proof of identity (Any of the following) - Original and photocopy of Birth Certificate
- Any legal or government issued document to prove his/her age and identity (SSS ID, PhilHealth ID, GSIS ID, TIN ID, Pag-ibig ID, Company ID)
3. Clearances (Any of the following) - NBI Clearance;Police Clearance;Court Clearance
4. Medical Certificate (from an LTO accredited or Government Physician)
5. Taxpayer's Identification Number (TIN)
6. Must have passed the written examination.

For Professional License(DOTC-LTO)

Qualifications :
1. Must be at least eighteen (18) years old.
2. Must be physically and mentally fit.
3. Must not be a drug user or alcoholic.
4. Must be able to read and write in Filipino or English.
5. Must be clean, neat and presentable.

Requirements :
1. Duly accomplished Application Form for Driver's License.
2. Valid Student Permit (at least five (5) months old / NP - 4 mos. old).
3. Medical Certificate (from an LTO accredited or Government Physician) or valid AFP/PNP organic personnel ID.

4. Negative Drug Test result (from DOH accredited drug testing center)

5. Taxpayer's Identification Number (TIN).
6. Clearances
-- NBI, Police or Court Clearance or Certificate from Commanding Officer, if a military personnel.
7. Must have passed the written and practical examination.

- Translation from Embassy/Consular Office (if license is not written in English)
2. Original valid Visa/Passport with photocopy (Foreigners)

Note: A foreign driver's license is not acceptable for conversion into Philippine driver's license if:
The foreign license is not written in the English language; orThe country of origin of the driver's license has no embassy in the Philippines

For Non-Professional

1. Must be at least seventeen (17) years old.
2. Must be physically and mentally fit to operate a motor vehicle

4. Must be able to read and write in Filipino or English
5. Must be clean, neat and presentable

Requirements :
1. Duly accomplished Application for Driver's License.
2. Valid Student Permit (at least one (1) month old)
3. Medical Certificate (from an LTO accredited or Government Physician) or valid AFP/PNP organic personnel ID
